We're working on behalf of a healthcare organization whose PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communitiesTheir innovative and comprehensive range of medical services to participants is what ignites the organization's passion to treat the whole person and not the symptomsThey employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to organizational values, team culture, and mission.
We seek a California licensed candidates with two (2) or more years of ambulatory clinic operation experience for the role of Clinical Nurse ManagerThe person in this role will operate a high-performing, well-equipped, and safe clinic, as well as ensure workflow efficiency, clarity, and protocolsShe/he will hire, retain, coach, develop, supervise, and ensure the RN Case Managers, Clinic RN, Medical Assistants, and other clinic team members are engaged and healthyShe/he will analyze data with attention to trends, strengths, weaknesses, and opportunities, performing root cause analysis, evaluating care delivery processes, and based on findings, making recommendations to the CSD on changes to strategies, workflows, and policies to continuously improve clinic operationsShe/he will ensure nursing and clinic staff deliver and correctly document interventions as agreed upon in participants' care plans, including but not limited to promptly and accurately responding to physician orders, completing assessments, , and actively participating in care planning for all assigned participants.
Candidates must have an Associate or Bachelor's degree in nursing and be licensed as an RN in the state of CaliforniaMust have a minimum of two (2) years of experience overseeing an ambulatory clinic operation including RN, LVN, and MA staff; equivalent nurse leader experience in a high volume, outpatient setting may be substitutedMust have at leasttwo (2) years of experience in a clinical setting with a frail or elderly population.
The role includes highly competitive compensation (salary to $152,000 based on experience) and Medical insurance coverage (Medical, Dental, Vision) as well as 17 days of personal time off (PTO), 12 holidays observed annually, and sick time401 K savings
