Social Worker (HUD VASH)

The duties of the Social Worker (HUD-VASH) may include but are not limited to the following:
Social worker utilizes a variety of clinical skills adapted to both inpatient and outpatient clinic settings and community visits and outreach; incorporates the VA ICARE values within the position functions assigned and use independent practice clinical skills for a specialized Veteran population where care is provided. Social worker performs general and specialized case management functions throughout medical center providing case coordination; planning and documentation to assure care provided meet objectives and compliance to VA Directives and Handbooks. - Coordinates services of Veterans assigned through case management and documents the overall acuity level and effectiveness of the services provided utilizing patient centered care approach to assure Veteran is involved in decisions regarding their healthcare. - Screen Veterans for eligibility; actively assist eligible Veterans in attaining and completing documentation for HUD voucher screening; coordinates with Public Housing Authority for HUD Voucher approval; once approved actively assist the Veteran in locating housing that accepts voucher within a designated timeframe; once housed, conducts a re-assessment in conjunction with the Veteran to determine the case management level of support (case management levels of care are identified in VHA Handbook Standards for VA Case Management). - Responsible for conducting visits and documenting appropriate to the identified acuity level case management; conducts on-going re-assessments with the Veteran as acuity level of follow-up may change. - Responsible to track and enter individual Veteran throughout the application for voucher, attaining voucher and housed using voucher. Timely and accurate data is reported nationally in the HOMES program and monitored as a program performance measure. - Participates in collaborative treatment planning with the Veteran, family and the multi/interdisciplinary treatment team; prior to team meetings, reassess the Veterans current psychosocial situation, to include financial, residential status, coordinate any equipment needs and other needs that may affect discharge. - Provide high-risk psychosocial assessment In crisis intervention, seeking to address the cause as well as the presenting complaint; communicating with the consulting team, Veteran family psychosocial resources available. Utilizes social work professional knowledge and experience as it relates to specialized Veteran medical and mental health diagnoses; disabilities and general treatment procedures. - General knowledge of Veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. Direct Patient Care Social worker independently completes functions with Veteran's enrolled. Caseload is defined program assigned. The level of case management contact is based on the acuity level and teamlet assigned, including the minimum or maximum contact, is variable based of level of care needed or defined by VA Handbook for case management practices and acuity level.Interventions take place in variety of settings and not necessarily in the traditional office setting. Travel throughout the medical center and the community is a component of providing patient centered care, continuity of care and for follow-up with the Veteran, families, community agencies adhering to SOP, Handbooks and Directives. The incumbent must have a current, valid Driver's License and be able to drive a government vehicle. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.Basic Requirements:
United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Social workers must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation). Grade Determinations:
In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker, GS-11. This is the full performance level for Social Workers. GS-11 Social Workers are licensed or certified to independently practice social work.(1) Experience and Licensure:
Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level.OR(2) Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Work is primarily sedentary. Typically, the employee will sit to do the work. However, there may be some walking standing, bending, carrying of light items, accessing transportation, and/or driving a government vehicle. The work requires patience and control of emotions. Work may require occasional use of appropriate techniques to physically restrain clients who present a danger to self or others.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
